According to CMS, the Annual Wellness Visit includes a “focused exam” not a head-to-toe exam. The minimum is height, weight, BMI and blood pressure. This can be member-reported or carried over from a previous visit during the same calendar year. If the health plan pays an incentive to complete a form, they can require additional elements not required by CMS.
However, it is generally acceptable to write “N/A” on portion of the exam that are not indicated. Many elements of the AWV can be completed by staff under provider supervision. The provider’s role is mainly to address the member’s preventive care and chronic conditions with an assessment/plan for each. This is very important for risk adjustment.
